KAISER PERMANENTE

Kaiser Permanente's Program and Resources

At Kaiser Permanente prevention is in our DNA. We offer hundreds of health and wellness classes at our medical offices where these services are integrated into every member's medical care.

Our prevention programs don't end with a members' visit to our facilities. At kp.org we offer members access to prevention programs and resources from home or work, 7 days a week, 24 hours a day. With the Total Health Assessment*, also known as a Health Risk Assessment, offered in partnership with HealthMedia, members can improve their health from the privacy and comfort of home or from work. (In order to take advantage of these programs, members must register to use kp.org.**)

The Total Health Assessment is a great place to start. Simply complete a brief online questionnaire. You will receive a personalized individual health guide or action plan after you click submit. You will also receive personalized, follow-up e-mails from HealthMedia designed to keep you on track.

Your tailored action plan directs you to further KP.org or HealthMedia Web-based interventions to help you achieve your health and wellness goals. The following Healthy Lifestyle programs are also available to Kaiser Permanente members at no additional charge:

  • Balance – for weight loss
  • Breathe – for smoking cessation
  • Relax – for stress reduction
  • Nourish – for better nutrition
  • Care for Your Health – for help with on-going health condition(s)
  • Care for Pain – for help with chronic pain

The Total Health Assessment is a great way to take control and begin to manage your health. On KP.org, it's just the start. With KP.org's My Health Manager - your personal health record (PHR) - you'll have access to features like e-mailing your doctor, viewing your lab test results, viewing your allergies and immunizations, seeing information about your past office visits, ordering prescription refills, scheduling appointments, and tools like health calculators to calculate your target weight or heart rate and much more. Unlike other personal health records, you won't just be able to view your health record, you will be in charge of it.

* Members must be at least 18 years or older to access the on-line Total Health Assessment and any of the on-line Healthy Lifestyle programs.

** In March 2008, registration will be streamlined so that members can register during their first visit to the site, if they are not already registered.
