The StayWell Program
The StayWell program includes: 
- Access to health information, tools, and calculators through your personalized member website
- Annual health assessment for each member
- NextSteps® Wellness Lifestyle Coaching program for those identified through health assessment with one or more high risks

Annual Health Assesment & Gift Card Program
The StayWell program is available to help you change your health behaviors and create a healthier lifestyle. To help get you started, UC will provide a $100 gift certificate to each eligible employee and retiree; and a $50 gift certificate to eligible spouses/domestic partners as an incentive for taking the health assessment & completing a StayWell follow-up program. Even if you or your spouse/partner completed a health assessment and received a gift card in previous years, you may participate again this year.
Who's Eligible
StayWell is available to employees, retirees, and their spouse/domestic partners (age 18
and older) who are enrolled in Anthem Blue Cross PLUS and PPO,
Anthem Lumenos, Core, Health Net, Health Net Blue & Gold, High Option, and Western Health
Advantage.
UC Kaiser Permanente members and their family members aged 18 and older are not eligible to participate in the StayWell health assessment, but they can take a free health assessment through Kaiser's HealthWorks program.
Some represented employees also may not be eligible for the StayWell health assessment program because participation was not agreed to during the collective bargaining process.
Eligible employees must be enrolled in a UC medical plan on January 1, 2013.
StayWell is not available to those who live outside of the United States.
How to Qualify
To qualify for the gift certificate, the health assessment must be completed by June 15, 2013 and the follow up program must be completed by December 15, 2013.
StayWell recommends that you get started early in order to ensure you have enough time to complete the recommended follow-up program. Depending on your individual situation, follow-up programs can take several weeks to complete.
