Take a New Health Assessment

Faculty, staff, retirees and their spouses or domestic partners who are enrolled in a non-Kaiser medical plan are eligible to complete the StayWell health assessment and participate in follow-up wellness coaching for which they qualify. More information is available on the StayWell website. Children and other family members (other than spouse/domestic partner) are not eligible for the StayWell program in 2010.

Employees represented by some UC unions may not be eligible for StayWell because participation was not agreed to on behalf of their members during the collective bargaining process.

UC Kaiser Permanente members and their family members aged 18 and older may take a free health assessment through Kaiser's HealthWorks program.

Please note, however, that the period for receiving an incentive for taking the StayWell health assessment or qualifying for a prize drawing for Kaiser's health assessment ended on April 15 for most employees.

Because the University and the Clerical & Allied Services (CUE) reached agreement to the terms of the StayWell wellness benefit late in 2008, the deadline for CUE members to earn an incentive for taking the StayWell Health Assessment is June 10, 2009. The incentive for completing StayWell's health assessment is a $100 gift card for faculty, staff and retirees and $50 for spouses/domestic partners, but CUE members must complete the health assessments by the June 10 deadline to qualify for the gift card.
