No. Kaiser Permanente offers the Total Health Assessment in partnership with HealthMedia, the global leader in scalable web-based behavior change interventions that produce outcomes. The Total Health Assessment is a fully integrated feature on Kaiser Permanente's web site with links back to KP.org features and functionality and the other suite of HealthMedia behavior change programs.
Are the recommended programs as a result from the Total Health Assessment provided by Kaiser Permanente?
The Total Health Assessment generates personalized recommendations, based on each member's unique responses, and guides them to the appropriate HealthMedia behavior change programs that are also available on KP.org. In addition, the Total Health Assessment and all HealthMedia Healthy Lifestyle programs have been customized by Kaiser Permanente with links to specific resources, features and functions on KP.org.
Are my individual Total Health Assessment results shared with the University?
No. No individual health information is shared with the University. Only de-identified summarized information may be shared with the University for the purposes of aggregate reporting and or wellness program planning.
Are my individual health assessment results shared with Kaiser Permanente?
Kaiser Permanente enables members to opt-in to send their assessment information directly to their electronic medical record, so the results can be shared with the medical care team. Any member may, however, freely decline to send their health assessment results to their electronic medical record.
